De rol van FGF23
FGF23 is een eiwithormoon dat serumfosfaatniveaus reguleert door zowel herabsorptie van fosfaat in de nieren, als de absorptie van fosfaat uit de voeding in de darm te onderdrukken. 6,7
Bij patiënten met XLH leidt een overmaat aan FGF23 tot chronische hypofosfatemie veroorzaakt door fosfaatverlies in de nieren en een lagere fosfaatabsorptie in de darmen. 1,12,13
- Kenmerkende laboratoriumbevindingen bij XLH-patiënten zijn lage serumfosfaatconcentraties en een lagere herabsorptie van fosfaat in de niertubuli, gecorrigeerd voor glomerulaire filtratiesnelheid (glomerular filtration rate, GFR) 1
